术前肝功能储备正常的肝癌患者肝切除术后肝功能衰竭的 危险因素研究分析

摘要


术前肝功能储备与肝切除术后肝功能衰竭及其他并发症的发生密切相关，由于目前对于术后肝功能衰竭的
发生机制的了解仍然有限，所以肝切除术后肝衰竭的定义尚未统一。综述了术前肝功能储备、肝癌肝切除术后肝衰
竭的定义、危险因素、肝功能评估方法。分析表明应重视术前肝功能储备并充分评估，术中采取优化手术方式控制
出血，保留足够的功能性肝实质，使术后剩余肝体积能够充分代偿，术后转入肝病 ICU进行诊治预防，从而降低肝
癌肝切除术后肝衰竭的发生率，进一步改善患者预后，使患者收益最大化。
