维生素D在管理乳腺癌辅助化疗副反应中的作用

摘要


乳腺癌辅助治疗虽能延长生存,却常伴随骨髓抑制、癌因性疲乏、神经毒性及生活质量下降等治疗相关不良反应,这些副作用直接削弱患者耐受并拉低日常状态。维生素D被视作一种多效甾体激素,除调控钙磷外,其在免疫调节、抗炎、抑制增殖及细胞保护中的角色也日益受到关注。系统梳理现有研究可见,在辅助治疗阶段,维生素D可能通过提高血清25-羟基维生素D浓度,经由免疫调节减轻骨髓抑制,借抗炎通路缓解疲乏,并以神经保护机制改善外周神经毒性,从而整体提升患者生活质量。不过,当前证据多来自小样本随机对照或观察性研究,干预方案安全但尚需进一步验证。鉴于维生素D与化疗副作用及生活质量存在多重关联,其在改善乳腺癌患者辅助化疗体验中的价值值得持续关注。
