以肠-脑轴为链接的痴呆与便秘共病作用机制研究现状

摘要


目的:研究表明肠-脑-轴在痴呆与便秘共病机制中起着主要链接作用,但其机制研究虽众多确缺乏系统的总结,二者共病患者的管理方案仍缺乏明确系统的证据指导,故本文以总结二者共病研究进展为后续临床共病管理方案提供系统依据为目的。方法:调查近五年该领域流行病学与实验室研究结果并加以分析及总结。结果:综合近五年研究结论得出长期便秘的确会使痴呆发病风险增加,反之,痴呆也会加剧便秘的恶化,这一过程主要以肠-脑轴为链接,通过神经病理蛋白沉积、肠道微生物群代谢失调、肠神经损伤三大主要机制共同作用,在人体形成全身炎症环境从而导致肠道屏障、血脑屏障及神经元损伤,使得痴呆与便秘共病患者的两种疾病形成恶性循环,互相加重对方病情进展,严重损伤其生活质量及生存周期。结论:基于上述结果,可得出在当今老龄化人口社会时代,老年人群作为两种疾病高发的群体,探索通过调整患者膳食方案来靶向改善痴呆患者的肠道功能,从而去延缓患者认知功能的衰退是有临床和生理病理学依据的,且这种方式具有打破上述恶性循环的治疗潜力,在临床医疗中便于实现,成本低效益大;未来针对该领域还应继续扩大针对二者共病的多中心流行病学研究进一步明确因果关联,同时深入二者互相作用的机制研究。
