手术显微镜图像采集系统在乳腺癌手术中快速识别癌细胞的应用
摘要
估新型手术显微镜图像采集系统(DDB-20)在乳腺癌手术中快速识别癌细胞的诊断效能。方法:回顾性纳入2025
年8月至2026年2月在江苏省肿瘤医院接受乳腺癌手术的93例患者,共计284份样本(肿块32,切缘86,淋巴结
166)。术中使用DDB-20行快速成像(ES),以石蜡病理(HE)为金标准,对比FS,计算诊断指标。结果:ES总体
灵敏度、特异度、准确性、AUC(95%CI)分别为0.955,0.996,0.989,0.975(0.939,1)。切缘评估特异度0.988,
阴性预测值1.000;淋巴结评估灵敏度0.857,特异度1.000。ES单样本耗时仅(3.35±0.48)分钟,显著短于FS的
(29.06±2.83)分钟(P<0.001)。ES与FS诊断一致性高(Kappa=0.973)。结论:新型手术显微镜图像采集系统可在
术中实现快速、准确的细胞级成像,尤其在切缘评估中价值突出,有望成为可靠的术中辅助工具。
