单侧椎弓根旁入路明胶海绵填塞球囊二次扩张PKP术治疗粉碎性OVCF的临床研究

摘要


目的:探讨单侧椎弓根旁入路明胶海绵填塞球囊二次扩张经皮椎体后凸成形术治疗胸腰椎粉碎性骨质疏松
性骨折(OVCF)的临床效果。方法:对2023年4月至2025年10月收治的符合研究标准的OVCF患者共80例,根据
CT和MR的影像学表现随机分为对照组(A组)40例及实验组(B组)40例两个组,A组给予常规PKP治疗,B组
采取单侧椎弓根旁入路明胶海绵填塞球囊二次扩张PKP术治疗,比较两组围术期资料、术前术后VAS评分、影像资
料、随访资料。结果:两组手术均顺利完成,未出现严重并发症,手术过程中血压、血氧饱和度等均正常。两组病
例随访3个月以上,两组患者术后6小时、第二天、第四天、1月、3月VAS评分均较术前显著减少(P<0.05)。影
像资料:术后3个月与术前相比较,两组术后椎体高度均显著增加(P<0.05),而Cobb角显著减小(P<0.05)。A
组骨水泥渗漏为30%,而B组为7.5%;两组差异有统计学意义(P<0.05)。结论:单侧椎弓根旁入路明胶海绵填塞
球囊二次扩张PKP治疗粉碎性OVCF能减少骨水泥渗漏率。
