护士主导超声可视化引导鼻肠管置入术在重症患者中的应用效果研究

摘要


目的:探讨护士主导的床旁超声可视化引导鼻空肠营养管空肠营养管置入术应用于ICU危重症患者的临床
安全性和实操有效性。方法:选取2025年6月—2026年5月中山大学附属第一(南沙)医院ICU收治、在临床治疗
中需放置鼻空肠营养管的43例重症患者作为研究对象,依据置管方法不同分为传统盲插组(n=22)和床旁超声可视
化置管组(n=21)。比较两组置管操作全程患者生命体征的变化、单次置管操作时间、置管效率(操作时长+置管
后定位确认时长)、置管相关并发症(呛咳、鼻黏膜出血、肠黏膜损伤、导管滑脱)发生情况。结果:两组病人置管
期间心率、血压、呼吸、血氧饱和度变化幅度无统计学差异(P>0.05);两组病人置管时间比较差异无统计学意义
(P>0.05);超声可视化置管组病人呛咳、导管滑脱及鼻黏膜出血发生率低于盲插组(P<0.05)。结论:相较于传统
盲插法,护士操作床旁超声可视化鼻空肠营养管置入法置管效率更高,减少置管带来的局部损伤与呛咳、脱管问题,
且操作便捷高效,可在床旁快速实施,能够为危重症患者安全、快速建立幽门后肠内营养通路。
