颅脑手术后颅内感染的防治研究进展

摘要


神经外科开颅术后中枢神经系统感染是神经系统的急危重症。中枢神经系统(CNS)由于受到脑-血屏障
的保护,并不容易发生感染。然而,开颅手术破坏了这种结构性保护,并增加了开颅手术患者颅内感染的风险。开
颅手术、脑外伤、脑脊液引流等破坏血脑屏障,促使感染发生。其致残率、致死率高,不仅会对患者造成极大的伤
害,而且给患者家庭乃至整个社会带来沉重的经济负担。脑脊液细胞学、病原学检验及颅脑影像学改变是颅内感染
重要的诊断依据,近年来一些新型检查项目例如:脑脊液乳酸、细胞因子、免疫学标志物测定也被应用于颅内感染
诊断及预后评价。本文结合文献对颅脑手术后颅内感染的发病机制、病因、时间节点、临床表现、诊断以及治疗几
个方面的研究进展进行综述。
