IDEAl-IQ技术联合功能评分对肩袖损伤的研究
摘要
诊断与严重程度分级中的应用价值。方法:纳入67例肩袖损伤患者,根据Sugaya分级分为A组(正常/肌腱病,15
例)、B组(部分撕裂,37例)、C组(小全层撕裂,7例)及D组(中/大全层撕裂,8例)。所有患者行肩关节MRI
检查,测量冈上肌、冈下肌、肩胛下肌及小圆肌的FF值,并收集Constant与ASES评分。比较组间差异并通过ROC
曲线分析各参数的诊断效能。结果:冈上肌FF值在四组间差异具有统计学意义(P=0.006),且随损伤加重而升高
(D组中位数5.63%)。Constant与ASES评分随损伤加重而显著下降(P=0.001)。ROC曲线显示,冈上肌FF值在鉴别
全层撕裂(C、D组)与非全层撕裂(A、B组)时效能良好(AUC=0.805),联合肩关节功能评分后诊断效能更优
(AUC=0.805,灵敏度73.3%,特异度75.0%)。结论:IDEAL-IQ定量测量的冈上肌FF值与肩关节功能评分密切相
关,二者联合能更全面、准确地评估肩袖损伤严重程度,为临床诊疗决策提供重要依据。
