原发性骨髓纤维化患者合并隐球菌性脑膜炎 1 例并文献分析

摘要


目的：分析原发性骨髓纤维化（PMF）患者合并隐球菌性脑膜炎的临床特点，提高血液病患者合并少见感染的诊治水平。
方法 ：回顾性分析 2022 年 5 月潍坊市中医院收治的 1 例合并隐球菌性脑膜炎的 PMF 患者的诊治经过，并复习相关文献。结果 ：
该患者确诊原发性骨髓纤维化，经系统治疗达临床改善。后患者出现头痛、烦躁不安等神经系统症状，提示存在中枢神经系统
感染可能，行脑脊液涂片及培养，确诊为隐球菌性脑膜炎，并给予抗真菌治疗，病情好转。结论 ：血液病患者若出现中枢神经
系统感染的症状，在排除恶性血液病中枢神经系统侵犯的前提下，应高度警惕隐球菌性脑膜炎的可能，并联合脑脊液培养及隐
球菌荚膜抗原等检测，以明确诊断，尽早规范治疗。
