基于模拟器的教学方法在软式支气管镜引导插管培训中的应用

摘要


目的:本研究旨在探讨基于支气管镜模拟器的综合教学方法在无支气管镜操作经验医生进行软式支气管镜引导插管(FOI)培训中的应用效果。方法:我们设计了一项前瞻性自身对照研究,纳入海军军医大学附属医院的35名学员,评估软式支气管镜引导插管培训效果。综合教学课程持续5周,共10小时,包括理论知识和基于模拟器的实践操作,由呼吸内镜专家授课。在培训前后,记录支气管镜从鼻腔进入可见声门和隆突、气管插管及通气的时间,并采用支气管镜操作全球评分量表(GRS)评估操作流畅度。结果:所有35名学员在培训后均能独立完成支气管镜引导插管。培训后FOI总时间明显短于培训前(46.20±2.66秒vs.68.00±8.06秒,p<0.01)。GRS评分也显著提高(5.51±0.49vs.15.51±1.60,p<0.01)。第4次和第6次模拟操作时间分别为22秒和21秒,差异无统计学意义。问卷调查显示,所有学员均受益于该模拟器的综合教学课程。结论:基于模拟器的教学方法在无支气管镜背景的医生FOI培训中具有显著实用性,值得推广。该基于支气管诊疗模型的模拟器可广泛应用于软式支气管镜引导插管培训,具有良好的安全性和可重复性。
